Hypertonic saline for treating raised intracranial pressure: literature review with meta-analysis A review

چکیده
210 J Neurosurg / Volume 116 / January 2012 INTRACRANIAL hypertension following neurological injury is often associated with poor outcomes.13 Increased ICP reduces CBF and can lead to brain herniation and death. The Brain Trauma Foundation has recommended that therapy to reduce ICP should begin at pressures > 20 mm Hg. Hyperosmolar therapy is a commonly used treatment for intracranial hypertension. Currently, only 2 agents are used for this purpose: mannitol and HTS. The Brain Trauma Foundation currently recommends mannitol as the mainstay in the management of intracranial hypertension, but HTS represents a potential alternative that is gaining favor.9 The reported concentrations of HTS for clinical use range from 2% to 23.5%.

Hypertonic saline for treating raised intracranial pressure: literature review with meta-analysis.
OBJECT Currently, mannitol is the recommended first choice for a hyperosmolar agent for use in patients with elevated intracranial pressure (ICP). Some authors have argued that hypertonic saline (HTS) might be a more effective agent; however, there is no consensus as to appropriate indications for use, the best concentration, and the best method of delivery. To answer these questions better, th...
متن کاملHigh-osmolarity saline in neurocritical care: systematic review and meta-analysis.
BACKGROUND AND PURPOSE Intracranial hypertension and cerebral edema are known contributors to secondary brain injury and to poor neurologic outcomes. Small volume solutions of exceedingly high osmolarity, such as 23.4% saline, have been used for the management of intracranial hypertension crises and as a measure to prevent or reverse acute brain tissue shifts. We conducted a systematic literatu...

Review and recommendations on management of refractory raised intracranial pressure in aneurysmal subarachnoid hemorrhage
Intracranial hypertension is commonly encountered in poor-grade aneurysmal subarachnoid hemorrhage patients. Refractory raised intracranial pressure is associated with poor prognosis. The management of raised intracranial pressure is commonly referenced to experiences in traumatic brain injury. However, pathophysiologically, aneurysmal subarachnoid hemorrhage is different from traumatic brain i...
متن کاملHypertonic saline versus mannitol for the treatment of elevated intracranial pressure: a meta-analysis of randomized clinical trials.
OBJECTIVES Randomized trials have suggested that hypertonic saline solutions may be superior to mannitol for the treatment of elevated intracranial pressure, but their impact on clinical practice has been limited, partly by their small size. We therefore combined their findings in a meta-analysis. DATA SOURCES We searched for relevant studies in MEDLINE, EMBASE, the Cochrane Central Register ...
